29 CFR 1910.146(k)(1)(i):The employer did not ensure that each member of the rescue service was provided with, and trained to use properly the personal protective equipment and rescue equipment necessary for making rescues from permit spaces: (a) On or about 28 January 2022, and in times prior thereto, the employer did not coordinate with the identified rescue service to ensure that said service was trained for rescue of entrants from permit required confined spaces while clearing jams inside of cooker shells
