29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. (a) Rivet Machine - On or about December 3, 2021 employees clearing a jam did not use a lockout device to isolate the air and electrical before clearing a jam, exposing employees to amputation hazards.
