29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of the conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose. A) Maintenance Shop - 1126 Richmond Ave, Staten Island, NY 10314 A spliced cord was energized through an outlet nearby the bench grinder and was available for use. The cord was used to provide power to nearby portable tools and equipment. The spliced wires were held together with black tape, which did not provide the same insulative properties as the original insulation, exposing employees to electric shock hazards. Exposure observed on or about 12/06/2021
