On or before October 18, 2021 at a worksite located at 42722 OH-7, Clarington, Ohio 43155 where employees were removing existing roofing materials, the employer failed to ensure the warning lines were at least 6 feet from the opening being removed: in that, the warning lines at the north east side of the opening were at or over the edge of the section being cut out. While two employees were cutting the existing decking, the victim stepped on the cut decking section when the decking gave way. The victim fell 38 feet, resulting in his death.
