29 CFR 1910.252(a)(2)(xv): Cutting or welding was permitted in areas that were not made fire safe by removing combustibles or protecting combustibles from ignition sources: On or about September 27, 2021 and at times prior thereto at the DA-101 Quench Tower in the Petro 2 Unit, workers were exposed to fire and explosion hazards when cutting and/or welding occurred in areas that were not made fire safe.
