29 CFR 1910.147(f)(3)(ii)(D):Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Heritage Cooperative, Inc. doing business as Heritage Cooperative LLC. located at 13118 Ohio 31 in Kenton, Ohio 43326: On or about September 1, 2021 the employer did not ensure that procedures were utilized while engaged in cleaning activities inside grain storage bins which affords each employee involved a level of protection equivalent to that provided by the implementation of a personal lockout device. Employees were exposed to engulfment and entrapment hazards in that each employee engaged in cleaning activities inside of grain bins did not apply a personal lock out tag out device.
