Text For Citation: 01 Item/Group: 002B Hazard:
1910.147(d)(5)(i) All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: On or about July 17, 2021 at the jobsite located at 3105 N. IBP Rd in Holcomb, Kansas 67851 employees engaged in maintenance activities where employees were exposed to amputation hazards associated with hazardous energy from gravity while working on the Stacker Dee. After following the procedural steps for locking and tagging out the Stacker Dee, an employee amputated the tip of their pinky finger on the chain that they were replacing in the Stacker Dee. The Stacker Dee became dislodged from its resting state, falling and catching the employees pinky in the chain. The maintenance staff failed to properly lock and tag out the Stacker Dee due to not determining gravity as a hazardous energy source.
