29 CFR 1910.179(j)(2): Frequent inspections of the cranes were not performed at intervals as generally defined in paragraph (j)(1)(ii)(a) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspection. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: a. Liberty Oilfield Services, Inc., 9540 E. 104th Ave Henderson, CO 80640: On or before July 21, 2021 the employer did not ensure that the AmQuip 15 ton Bridge Crane being used in the workplace had an effective warning visual signal to notify employees in the working area that the crane was in motion. An employee who did not know the crane was moving, was struck by the bridge of the crane while working on top of a Hydraulic Horse Power-Pump Truck and fell over 13 feet to the ground. This condition exposed employees to struck-by and fall hazards.
