29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about May 26, 2021, and at times prior thereto, employees were required to performing maintenance on machines within the facility without the use of locks and tags, exposing employees to hazards such as, but not limited to the release of stored energy and unexpected startup.
