29 CFR 1926.1060(b): The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training: (a) 1021 Mount De Chantel Rd, Wheeling, WV - On or about May 12, 2021, the employer did not retrain employees who were accessing the roof using multiple extension ladders which were improperly set up, exposing employees to fall of up to 11 feet.
