29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: a) Case Dock, Containment Area: On or about April 29, 2021, and at times prior, employees were exposed to slips, trips, and falls while working in and around a containment area that was not properly guarded. The hole was approximately 78 inches long by 21 inches wide by 5 inches deep.
