29 CFR 1910.119(e)(6): The employer did not update and revalidate the requirements in paragraph (e)(4) of this section at least every five years, to assure that the process hazard analysis is consistent with the current process. On or about April 9, 2021, Oberweis Dairy, Inc. did not perform a process hazard analysis (PHA) at least every five years to ensure consistency with the ammonia refrigeration process. Employees and contractors are exposed to inhalation and burn hazards caused by ammonia releases throughout the facility. There is no abatement certification or documentation required for this item.
