29 CFR 1910.146(d)(6): The employer did not provide at least one attendant outside the permit space into which entry is authorized for the duration of entry operations: (a) Dick's Sporting Good, Inc. at 31 W Flatiron Crossing Drive in Broomfield, CO: On or before March 30, 2021, the employer did not provide at least one attendant outside the permit space into which entry is authorized for the duration of entry operations. The compactor met the definition of a permit space, and operations employees regularly entered the space while safety hazards were present, but the employer did not ensure requirements for permit space entry were implemented. This condition exposed employees to crushing hazards.
