29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources: On or about December 7, 2020, employees conducting maintenance on machine 4065 were exposed to being caught-in the machine die, in that the electrical, hydraulic, and pneumatic energy sources were not isolated.
