29 CFR 1926.850(c): All electric, gas, water, steam, sewer, and other service lines were not shut off, capped, or otherwise controlled, outside the building line before demolition work was started: A.) B&B Designed Systems, Inc., 18746 Mill Street, Meadville, PA 16335, on or about September 18, 2020 and times prior thereto: An employee was removing electrical components including, but not limited to, de-energized 480-volt wiring, low-voltage control wiring and conduit. In the process of cutting the low-voltage control wiring, the employee cut through a concealed 480-volt wire that was energized. The 480-volt wiring was not de-energized prior to cutting the low-voltage wiring for removal of the wiring and associated conduit. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
