29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection. On or about September 29, 2020, on the west side of the project site; Employees were preparing to perform sheet metal roofing repair on a 4:12 sloped roof at heights upwards of twenty-five (25) feet without lanyards on each of their respective harnesses.
