29 CFR 1910.147(c)(4)(ii)(C): Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; a) At the facility, on or about September 3, 2020, the employer failed to develop specific procedural steps for the vertical hydraulic levelers, in that they contained three points for lockout / tagout and maintenance personnel were only provided two locks and tags, thereby exposing employees to crushing, struck-by, and caught-in hazards.
