29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: A) Morral Companies, LLC, located at 132 Postle Avenue, Morral, OH 43337: On or about April 16, 2020, the employer did not document the completion of the measures required for safe permit space entry. The entry permit was not completed prior to entering into the Lawn Mixer located in the Liquid Department, exposing employees to thermal, atmospheric, engulfment, engulfment, and mechanical hazards.
