29 CFR 1910.119(e)(3)(v): The process hazard analysis did not address the hazards related to facility siting: (a) Fruit Cooler Storage Room: On or about April 3, 2020, the employer located storage racking in close proximity to small diameter liquid ammonia lines, adjacent to evaporator AAU-7, without providing adequate protection from rack loading and unloading activities. This hazardous condition exposed workers to toxic and corrosive hazards when a powered industrial truck struck ammonia piping resulting in an anhydrous ammonia release.
