29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. a. On or about March 6, 2020, and at times thereto, employees operating Laminator 4 were exposed to an unguarded protruding shaft end on the Foil Unwind shaft and Paper Unwind shaft. b. On or about March 6, 2020, and at times thereto, employees operating the Johnstown Slitter were exposed to an unguarded protruding shaft end on the bottom roller of the Web Guide section. c. On or about March 6, 2020, and at times thereto, employees operating the Laminator 7 were exposed to an unguarded shaft end on Foil Unwind 1 shaft.
