29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) On or about 02/13/2020 in the plant 17 model shop; where the area supervisor applies a lock to the band saw then different employees change the blade on the band saw. The employees performing the work, the "authorized" employees, were not the employees who applied the lock to the energy isolating device. ABATEMENT CERTIFICATION REQUIRED
