29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: On or about January 20, 2020, the employer failed to ensure specific energy control hardware was provided/available when working on the wheeled Caterpillar loader model 938M. The employees were trying to repair a suspected hydraulic hose line leak. Employees were exposed to an unexpected start-up and/or unexpected movement of the loader. Employees were exposed to a crush-by hazard.
