29 CFR 1926.852(f): Where material was dumped from mechanical equipment or wheelbarrows, a securely attached toeboard or bumper, at least 4 inches thick and 6 inches high, was not provided at each chute opening. (a) Madison Elementary, Wheeling, WV - On or about 12/16/19, employees were dumping debris from wheelbarrows into a chute opening which did not have a toeboard or bumper, exposing employees to fall hazards.
