29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, it was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: Instances including, but not limited to, the following: a) Job site, 33 McGovern Road, Crescent, PA: On or about December 13, 2019, during production activities at the Verson Press Brake number 522, serial number 13349-1062, located in building 4, the employer did not ensure that the 2 hand controls used for machine guarding for the unit were appropriate, exposing employees to caught in hazards. The employer did not have them installed in a location based on a determined safety distance from a machine stopping time and the controls allowed you to let go of one palm button and then reactivate it to make the machine operate rather than having to let go of both buttons first.
