29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Cincinnati Shear: On or about November 22, 2019, employee(s) using the shear to cut sheet metal, exposed the operator to crushing hazard and amputation injury, in-that sufficient guarding was not in place to prevent the employees fingers, and hands from coming into contact with the shear blade point of operation.
