29 CFR 1926.502(d)(16)(iv): Personal fall arrest systems, when stopping a fall, did not bring an employee to a complete stop and/or and did not limit maximum deceleration distance to 3.5 feet (1.07 m): On or about November 19, 2019 employee was attached to a personal fall arrest system that was anchored below the employee and fell greater than 3.5 feet from a roof onto a scissors lift basket floor.
