29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about November 12, 2019, and at times prior thereto, employees in the machine shop were required to utilize metal lathes to work on long steel shafts, specifically to smooth them out and reduce their outer diameter. Employees were instructed to manually sand the exterior of steel shafts that were unguarded and actively rotated by metal lathes at speeds of up to 500 RPM, exposing employees to the hazard of becoming caught in rotating parts. The Materials Transportation Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.212(a)(3)(ii)), which was contained in OSHA inspection number 1164556, citation number 1, item number 2, and was affirmed as a final order on September 2, 2016, with respect to a workplace located at 1408 S. Commerce St., Temple, TX 76504.
