29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that were needed to control the energy to the machine or equipment were not operated in such a manner as to isolate the machine or equipment from the energy source(s). On or about November 14, 2019, at the Tri-State Iron and Metal Company in Texarkana, Arkansas; the employer did not ensure that the equipment was isolated from its energy source(s) when clearing debris was performed on the conveyor belt system.
