29 CFR 1926.451(b)(2): Each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide. On or about November 5, 2019, and at times prior thereto, employees were building makeshift scaffolds to remove form wood from the concrete wall. Scaffold platforms were seven to nine inches wide without load ratings, exposing employees to a fall hazard.
