In building 10, on the north wall where an employee was exposed to caught-by and struck-by hazards while using a Marvel brand vertical saw, company ID G613, without a point of operation guard covering the unused portion of the blade. b. In building 10, west center area of the shop, where an employee was exposed to caught-by and struck-by hazards while using a Bridgeport brand vertical mill, company ID R222, without a point of operation guard. c. In building 10, west center area of the shop, where an employee was exposed to caught-by and struck-by hazards while using a Bridgeport brand vertical mill, company ID R223, without a point of operation guard. d. In building 10, north wall west end area of the shop, where an employee was exposed to caught-by and struck-by hazards while using a Bridgeport brand vertical mill, company ID G224, without a point of operation guard. e. In the weld shop, northwest corner of the shop, where an employee was exposed to caught-by and struck-by hazards while using a Johnford brand drill vertical mill (model 4AV3, serial number 7Y30CR) without a point of operation guard. f. In the weld shop, west wall center area of the shop, where an employee was exposed to caught-by and struck-by hazards while using a Fosdick brand drill (no serial number) without a point of operation guard.
