29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard: a) Jobsite located at 75 McAvoy Lane, Phoenixville, PA. 19460, storage area above holding room: The employer did not ensure that employees were protected from falling into the holding room which contains mobile kiln cars. An employee accessing the storage area above fell approximately 10 feet through the ceiling and was injured. Condition was noted to exist on or about 9 AUG 2019.
