29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about July 10, 2019, the employer did not ensure that a personal fall arrest system (PFAS) or travel restraint was used by an employee who was working from the basket of a rented articulating boom lift (JLG 400s). The employee was tasked with servicing an industrial baler, and was exposed to a 13(+) ft. fall.
