29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: a) On or about 05/21/2019, in the Loading dock (Door #2), employees were exposed to electrical shock when they plugged in the portable conveyor belt the wrong way in the outlet (90 degrees counter-clockwise from the correct orientation) energizing the grounding conductor which made the portable conveyor belt energized. Employee then received an electrical shock when he attached the conveyor to the trailer because the frame of the conveyor belt was energized. NO ABATEMENT CERTIFICATION REQUIRED
