29 CFR 1926.1203(e)(2)(iv): A hazardous atmosphere was permitted within the space while an employee was inside the space. (a) At the construction site on Belfast Keller and Interstate I-95, on or about July 8, 2019, the employer allowed employees to enter an underground water main vault with an oxygen content of 12 to 16 percent, exposing employees to an oxygen deficient atmosphere.
