29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: On, or about, March 14, 2019, the employer did not ensure that each employee at the New South High School Construction Project located at 5000 Shankland Road, in Willoughby, Ohio, 44094 was trained in the use and operation of personal fall arrest systems while engaged in cleaning tasks, such as trash receptacle emptying near a second floor, unguarded opening, when elevated 15 feet above ground level and expected to use a personal fall arrest system.
