29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about 4/29/2019, although the company had established lockout/tag out procedures, they were vague and did not include information such as but not limited to the following: a. Equipment identifiers (e.g.: model number, serial number, company number of the machinery, etc.); b. The energy sources used to power the equipment (e.g.: electrical, pneumatic, gravity, etc.); c. The means to isolate, block and secure the machinery .
