29 CFR 1926.502(f)(2)(i): The rope, wire, or chain was not flagged at not more than 6-foot (1.8 m) intervals with high-visibility material: Instances including, but not limited to, the following: a) Job site, 1220 Western Avenue, Pittsburgh, PA: On or about April 22, 2019, during roofing activities on the main roof of the building, the employer did not ensure that flag lines were provided with high visibility material at no more than 6 foot intervals in that at least one area was measured to be approximately 8 feet in length and had no high visibility material on it.
