29 CFR 1910.29(b)(2): The employer did not ensure that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high: Warehouse second floor- On or about April 17, 2019, employer did not ensure that the mid rail was 21 inches when a chain installed on the second floor was sloped exposing employees to falls to concrete below when they accessed the second floor to get supplies and other items.
