29 CFR 1926.502(d)(20): The employer did not provide for prompt rescue of employees in the event of a fall or did not assure that employees were able to rescue themselves: a. On or about March 4, 2019, at the west gun barrel tank, the employer did not provide for prompt rescue of two employees attached by personal fall arrest devices to an elevated extensible boom lift basket that was greater than 6 feet from the ground. The employees fell from the west gun barrel tank top and were unable to self rescue.
