29 CFR 1910.30(a)(3)(iv): The employer must train each employee in at least the following topics: the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer. On or about 3/3/2019, on the tree at the rear of the residence located at 4374 Pleasant Forest Drive, Decatur, GA: An employee engaged in tree cutting and climbing was exposed to a fall hazard in excess of 25-feet in that training on the correct used of personal fall protection system and proper tie off techniques was not provided.
