29 CFR 1926.602(b)(3): Safety requirements, ratios, or limitations applicable to machines or attachment usage covered in Power Crane and Shovel Associations Standards No. 1 and No. 2 of 1968, and No. 3 of 1969 were not complied with: (a) Location - 94 Washington Square East, Salem, MA: On or about 2/15/2019, the operator of a Takeuchi Model TB007 Mini-Excavator Serial #1074544 was not provided with a cab constructed, insofar as practicle, without interruption of operation, to provide necessary protection of the machinery and operator's station from the weather as required by Section 8.1.8 of Power Crane and Shovel Association Standard No. 3 - 1969.
