29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about January 23, 2019, at 1039 Gulf Drive, Summerland Key, Florida, 33402, the employer did not ensure personal fall arrests systems used by employees applying roofing materials onto a steep slope (6:12) roof were inspected, in that employees were using a personal fall arrest system in which one of the lanyards was previously subjected to impact loading, and three of the lifelines were damaged. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
