29 CFR 1926.454(b)(3): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining or inspecting trained by a competent person in the design criteria, maximum intended load-carrying capacity and intended use of the scaffold. This violation occurred on December 18, 2018 at the 115 E. 5th St., Tulsa, Oklahoma worksite: Employees involved in the dismantling of the transport platform system were not adequately trained in the proper configuration and weight distribution required by the manufacturer to ensure a safe operation exposing employees to the hazards of falls.
