29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: a) Adelaide Mills site, 3rd floor, Southeast corner. - On or about December 10, 2018, the employer did not cover floor holes created by prior demolition or facility degradation, but chose to place door barriers marked as "DANGER - DO NOT ENTER" in combination with yellow CAUTION tape.
