29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she could contact cannot come closer to any unguarded energized overhead line: A.) Brad's Fuel Filtering, Inc., 801 Butler Pike, Mercer, PA 16137, west side of the prison facility, on or about December 13, 2018 and times prior thereto: An employee raised a metal stinger/siphon tube in the air and came into contact/proximity with 14.5-kV overhead powerlines. The employee was subsequently electrocuted.
