29 CFR 1910.218(a)(2)(ii): There were no scheduled and recorded inspections of guards and point of operation protection devices at frequent and regular intervals for forging machine(s): On or about November 5, 2018, employees were exposed to entanglement hazards and the employer did not schedule and record the inspection of guards and point of operation protection for the Bronx Tube Straightener (Model 2 13750) which included the following: a. date of inspection; b. signature of the inspector; and c. serial number/other identifier.
