29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. On or about October 1, 2018, employees were exposed to machine hazards associated with unexpected startup and the release of stored energy when performing service and/or maintenance activities on AMP 11, SMG 14, SMG 15, AMP 16, AMP 17, and SMG 19. The employer's LOTO procedures for these presses did not include safe shutdown procedures, isolating ram/blocking gravity i.e. slide locks for the control of hazardous energy.
