29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: A) Autoplas Division, Windsor Mold Group, located at 560 Goodrich Road, Bellevue, OH 44811: On or about September 10, 2018, the employer did not adequately train employees to lockout/tagout the hydraulic 2200 Ton Engel Press # 16 (Model: Duo 2200; Serial # 178920) and the Fanuc Robot (Type: R2000iA/165F) at the Press # 16. Employees did not control all hazardous energy sources prior to conducting activities such as, but not limited to, checking part numbers, connecting air lines, checking the valve gates, troubleshooting the robotic arm, and cleaning inside the robots cage. Employees were exposed to struck-by and caughtin-between hazards while performing these tasks. B) Autoplas Division, Windsor Mold Group, located at 560 Goodrich Road, Bellevue, OH 44811: On or about September 14, 2018, the employer did not adequately train employees to lockout/tagout the hydraulic 1000 Ton Demag Press # 13 (Model: Titan 900; Serial # 0508) and the Fanuc Robot (Type: R-2000iB/165F) at the Press # 13. Employees did not control all hazardous energy sources prior to conducting activities such as, but not limited to, rearranging air lines inside the mold and working inside the robots cage. Employees were exposed to struck-by and caughtin-between hazards while performing these tasks.
