29 CFR 1910.119(l)(2): The procedures did not assure that all of the following considerations, including the technical basis for the proposed change, the impact of the change on safety and health, and any resulting modifications to the operating procedures: a) 626 Lynnway, Lynn, MA On or about August 27, 2018, change management procedures did not assure that the splitting, and subsequent closure of the production side of the ammonia system, hazards associated with splitting the system were identified, and procedures to run the newly modified system were in place. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c)
