29 CFR 1926.416(a)(3): Employer did not advise employees of the location of energized electrical power circuits, the hazards involved, and the protective measures to be taken: (a) Gilbert Elementary School, Gilbert, WV: On or about August 23, 2018 the employer had not advised affected employees on the protective measures to be taken, such as de-energizing equipment, personal protective equipment and protective boundaries, prior to allowing employees to hook up a newly installed HVAC system to an existing 480 volt circuit breaker box.
